Any personal information acquired is held in written form for my own notes , insurance compliance purposes and kept in a locked file. Information is not shared with anyone and not kept for any longer than required. On the rare occasion I feel there is a requirement to refer to someone else including a GP, this will be discussed in full prior to any referral.
